Secondary School Documents
All of the below documents are needed for you to be able to commence with the Active Lives Survey. Sport Birmingham will also send these documents to you via email once you have agreed to take part in the survey.
Information for Parents
The below information is to be given out to parents/guardians. Please ensure that this information is given to parents with enough time before the survey is due to be administered.
- Letter for Parents Years 3 to 11 (MS Word, 26 Kb)
This letter is to be given to all parents/guardians of the students in the chosen classes from Years 3 to Year 11. Please ensure that you have had it signed by your headteacher or yourself and has your school letterhead on it.
The letter informs the parent/guardian about the survey that their child will be undertaking and allows them the option of taking their child out of the survey.
Please ensure that all parents receive this letter before you administer the survey.
Example Questionnaire
The below is an example of the survey that your students will be taking, please feel free to show these to students and class teachers before they take the survey so they are prepared.

School Info
Information below is to share with staff and students involved in taking the survey.
- Info For Secondary School Pupils (MS Word, 99 Kb)
This document will give the students an idea of what the survey is about and what is being asked of them. Show this to the students before the survey so they have an idea of what to do.
- Info for Teacher Completing the Teacher Survey (MS Word, 53 Kb)
This is information for the teacher that is taking the survey themselves. The teacher taking the survey must be involved in the PE delivery in the school.

Please ensure that the teachers of the chosen classes have read this document before administering the survey, it should assist them with questions surrounding the survey and how to administer it.
